HOW THIS MEDICATION WORKS

We need red blood cells to carry oxygen to our tissues and to remove waste gases from our tissues. If the number of red blood cells drops too low, we cannot carry out these functions and we become weak. If the number of red blood cells drops even lower, we may not have enough to stay alive. Fortunately, our bodies have hypoxia sensitive genes which turn on when low blood oxygen levels are perceived and special proteins are manufactured to step up red blood cell production. One of these proteins is called "hypoxia Inducible Factor."

Hypoxia Inducible Factor does its job and when it is no longer needed, it is ultimately removed from the circulation by special enzymes. Molidustat inhibits these enzymes thus prolonging the duration of Hypoxia Inducible Factor. More Hypoxia Inducible Factor means more red blood cell production and more iron mobilization to facilitate production of hemoglobin, the material red blood cells need to move gases through the bloodstream.

In short, molidustat helps the patient make more red blood cells and is presently only labeled for cats.

 

HOW THIS MEDICATION IS USED

If we have too many red blood cells, the blood becomes too thick and clots too easily. The goal is to have the right amount of red blood cells. Molidustat is meant to be used for a 28 day course but the patient will require some red cell checks during that time. The medication is discontinued when the appropriate red cell amount is achieved but will probably need to be restarted later on when red cell numbers decline again.

Store at room temperature away from light and use the dosing syringe provided.

SIDE EFFECTS

Approximately 50% of cats using this medication will have an increase in vomiting. This is usually not severe. Of more concern is an increase in red blood cell number that is so great that the blood thickens and can clot abnormally. High blood pressure can result as well. Monitoring the red blood cell count periodically will be important in preventing these situations.

INTERACTIONS WITH OTHER DRUGS

Antacids including sucralfate can interfere with the body's absorption of molidustat.

 

CONCERNS AND CAUTIONS

  • Molidustat should not be used in cats with a history of seizures nor in cats with a predisposition to abnormal blood clotting.

  • Molidustat should not be used in cats under one year of age.

  • Wash hands after handling this medication.

  • If a cat has not responded to molidustat after 3 weeks, it is important to look for a reason such as on-going blood loss or iron deficiency.

  • After treatment has been stopped, wait at least 7 days before re-starting. Treatment will be on and off so as to create an appropriate red blood cell count.

 Shake well before each use.
